Becoming A New Patient
To become a new patient, or to obtain additional information about our practice, please fill out the form below or contact Monica at 203-298-9005 ext.1 or [email protected] .
Recommended items to include in your inquiry:
- Service(s) you are seeking (i.e. medication management, psychotherapy, QBTest)
- DOB
- Insurance
- Office preference - Woodbridge, Hamden, or telehealth
- Day/time preference
- Presenting problem
If you are seeking medication management, you will be required to complete a pre-screen form. This is a one-page form that our prescribers review to determine whether or not they are an appropriate fit for the patient's needs.

Please note: If we do not immediately have someone available, we would be happy to add you to our wait list and/or to provide you with resources in finding a provider in the area.
New Patient - Next Steps
Once you've been assigned a provider, please send us the following information:
- Form of Identification (i.e. Driver's license, school card)
- Insurance card - Front and Back
- New patient packet
- Credit card authorization form (Note: Medicaid patients are not required to complete this form.)
- Patient Financial Statement form
- Telehealth consent form
